Requesting Transcripts
Students who attended the campuses of Ivy Tech Community College, Northwest Region, may obtain transcripts by mailing or faxing a written request to:

A transcript request form can be downloaded here
or you can write a letter of request, which must include:
Name (both current and while in attendance, if the two are different)
Address
Social Security Number
Year of last attendance or graduation
Address where transcript should be mailed
Student signature and date
If you attended another Ivy Tech campus or campuses in addition to Ivy Tech Northwest, the request must indicate the campus(es) and dates of attendance.
If you did not attended the Northwest Campus, go to www.ivytech.edu/campuses/ for additional campus addresses.
Students and former students can also request their transcripts in person at the Gary Campus, Monday –Friday, 8 am – 5 pm. Requests made by internet, telephone, or by other means will not be honored. Transcripts will not be issued via fax.
Transcript Fee
Effective August 22, 2005, all current and former students will be entitled to one transcript at no charge. Thereafter, a $5.00 per transcript fee will be charged and must be paid before a transcript will be issued.
The College is unable to honor transcript requests if you have an unpaid financial obligation to the College
